Expanding comprehensive medication management considerations to include responses to the social determinants of health within the BD Helping Build Healthy Communities Program

WAHIAWA, Hawaii, (KHON2) — Wahiawā Center for Community Health, commonly known as Wahiawā Health, announced that they are creating a new geriatric specialty center.

The new center will focus on optimizing healthcare for aging kūpuna in an effort to promote better health and quality of life for seniors.
Staff are working to identify high-risk patients and develop care plans that address obstacles to getting treatment. That includes language barriers and economic challenges, as well as food, education and environmental factors.

The Wahiawā Center for Community Health recently opened a diabetes specialty center to address the high rates of diabetes and prediabetes in the islands.

As a Federally Qualified Health Center Look-Alike, Wahiawa Health understands the troubling trends for access to health care as outlined in a recent health care survey report, and we are working hard to be a part of the solution (“Health care access in isles bleak, report says,” Star-Advertiser, July 27).
