Earn $50 per child for a well-child exam
The state Department of Human Services is reminding families enrolled in Med-QUEST that they may be eligible for a $50 incentive for each child who completes a well-child examination. The child does not need to be a Med-QUEST member.
Visit Wahiawā Health for a well-child exam, submit the DHS1193 Form that Wahiawā Health will provide, and a $50 incentive card will be mailed within 4–6 weeks. Call Wahiawā Health at 808-622-1618 to make a well-child exam appointment.
