Innovations in Care Award
We provide award-winning services! Wahiawā Health, in collaboration with our pharmacists, community health worker, population health coordinator and medical team was awarded the Innovations in Care Award from the BD Helping Healthy Communities Program! Thanks to private and philanthropic grant funding, we have created an integrated care team and care model to address chronic disease within our Native Hawaiian, Asian and Pacific Islander communities.
This team approach seeks to improve medication adherence to prevent health complications associated with chronic disease and cultural, social, economic, and environmental challenges such as poor health literacy and a lack of food and housing. The addition of a population health coordinator provides a dedicated resource for collecting and tracking more in-depth data on SDOH, hemoglobin A1c, health screening tools and comprehensive diabetes services throughout the year so that we may tailor our services to meet the needs of our community.

To reduce polypharmacy and drug adverse events, Wahiawā Health Pharmacy offers free pill pouch packaging that organizes all medications in a simple pouch, rather than multiple plastic pill bottles. It is easy to open and has large print for easy-to-follow directions.
Do I have to be a patient at Wahiawā Health to receive pharmacy services?

What is a non-profit pharmacy?
Unlike larger pharmacies and mainland owned chains, all funds received by our pharmacy go directly back into our health center to support jobs, programs and community outreach services that are often free to our community members. Wahiawā Community Health Center does not partake in profit sharing, nor do we have shareholders. Each prescription you fill helps us to fulfill our mission!
